Why I'm telling you this

"How can testosterone lower my fertility if it's precisely the male hormone?" It's one of the reactions I hear most often in my practice. And it's a logical question.

But the answer is surprising: taking testosterone from the outside can significantly suppress sperm production. Sometimes, without anyone having warned you.

I am Dr. Florian A. Vallecillo Cabrera, and today I want to explain this to you calmly, because if you are thinking about having children — now or someday — this conversation needs to happen before starting any treatment.

Imagine a thermostat

Testosterone production and sperm production are governed by a circuit connecting the brain to the testes. The brain sends hormonal signals that tell the testes two things: produce testosterone and produce sperm.

And it works like a thermostat. When the brain detects that there is enough testosterone in the blood, it turns down its signals to avoid overproduction. When it detects a shortage, it turns them up. A delicate balance, self-adjusting.

What happens when testosterone comes from outside?

When a man uses external testosterone — in gel, injections, or any other form — the brain detects high levels and does what makes sense: it shuts down its own signals. 'There's already plenty; let the testes rest.'

Here is the catch. Those very signals that the brain shuts down are the ones that kept the sperm factory inside the testes running. By silencing them, production slows or stops.

And there is a detail that almost no one knows: producing sperm requires extremely high levels of testosterone inside the testes themselves — far higher than those found in the blood. External testosterone does not recreate that internal environment; on the contrary, by shutting down the brain's signals, it makes things worse.

The result? Many men who use external testosterone see their sperm count plummet, sometimes to very low or zero levels.

And does this reverse?

In many cases, once treatment is stopped, production returns over time. But that recovery can take months, even more than a year, and it is not always complete or predictable. It depends on how long it was used, the dose, and your baseline situation.

That is why I never take 'it'll sort itself out later' for granted. When fertility is at stake, we do not gamble on reversibility.

The most dangerous misconception

At its core, there is a misunderstanding: many people lump 'testosterone,' 'virility,' and 'fertility' together, as if they were the same thing. They are not. External testosterone can maintain or even improve certain things… while, at the same time, it is suppressing sperm production.

I have seen men who were looking to 'boost their masculinity' discover, right when they wanted to become fathers, that their fertility had been significantly reduced. No one had told them this part.

What if there is a real deficit and I also want to have children?

Then the approach changes completely. There are medical strategies designed to treat the symptoms without fully shutting down the testes' factory, as well as the option of preserving (freezing) samples before starting certain treatments.

I won't go into specific medications here, because every case is different and should be managed by a professional. The message is that there are alternatives, and that planning ahead changes everything.

What I want you to remember

External testosterone can significantly reduce male fertility because it shuts down the brain signals that maintain sperm production. And that decline is neither always rapid nor always fully reversible.

So if you use or are thinking about using testosterone and you want to have children — today or in the future — talk to your doctor before you start. A good decision, made at the right time and with the right information, will spare you a regret that is very hard to undo later.
