Can a very common virus trigger skin cancer?

Why I'm telling you this

You have probably heard of the human papillomavirus — HPV — especially because certain types can cause cervical cancer, anal cancer, or some throat cancers. But there is another, far less well-known family: the beta-papillomaviruses, or beta-HPV.

These are extremely common viruses that normally live on our skin: many of us carry them without knowing it, and under ordinary circumstances our immune system keeps them perfectly under control. But an extraordinary case has just been described that compels us to understand their relationship with skin cancer a little better.

The case that makes us look again

A woman of just 34 years of age developed a cutaneous squamous cell carcinoma on her forehead — one of the most common skin cancers. And it was no trivial tumour: it was invasive, it kept coming back after treatment, and it eventually became extremely difficult to control.

What did they find inside the tumour?

The researchers analysed the tumour in depth and discovered something remarkable: a specific type of beta-papillomavirus, HPV-19, had integrated itself into the DNA of the tumour cells and was producing viral proteins. In other words, the virus was not simply present in the skin — it was directly participating in the maintenance of the cancer.

Does this mean that having HPV in the skin causes cancer?

No, and this is very important. This patient also had a very rare genetic alteration affecting a protein called ZAP-70, which is essential for our T lymphocytes to function correctly. Her defences may have appeared relatively intact, but her T lymphocytes had difficulty properly recognising and controlling certain HPV types. The virus was then able to persist, evade the immune system, and ultimately participate directly in the transformation of skin cells.

The most fascinating part: restoring immunity

The doctors performed a haematopoietic stem cell transplant to restore an immune system capable of responding correctly. After the transplant, the HPV-related conditions disappeared — including the squamous cell carcinoma — and no recurrence had been observed after more than three years of follow-up.

What does this teach us about skin cancer?

When we talk about skin cancer, we immediately think of the sun and ultraviolet radiation, and we are still right to do so: ultraviolet radiation remains one of the main factors responsible for cutaneous squamous cell carcinoma. But carcinogenesis is far more complex. In certain individuals, genetics, the immune system, viruses, and ultraviolet radiation may all act together.

We have known for some time that immunosuppressed patients — particularly certain transplant patients — face a much higher risk of developing cutaneous squamous cell carcinomas. This case now provides a particularly compelling demonstration: under certain conditions of impaired cellular immunity, a beta-HPV can stop behaving simply as a routine microorganism on our skin and begin participating directly in the development and maintenance of a cancer.

An important message: do not be afraid of HPV on your skin

Finding a beta-HPV does not mean you will develop cancer, nor does it mean we should start screening for these viruses across the general population. This is an exceptional case, associated with a very particular immune defect.

What truly matters remains knowing our skin, protecting it adequately from the sun, and seeking medical advice when a new lesion appears, a wound that won't heal, or a lesion that grows, bleeds, or changes in appearance. Because in skin cancer, as in so many other diseases, understanding the risk is important — but detecting it in time can change everything.

What to remember

  • The best-known HPV (associated with cervical, anal, and throat cancer) is not the main character here: there are the beta-papillomaviruses (beta-HPV), very common viruses that normally live on the skin and that the immune system keeps in check.
  • In an exceptional case, a 34-year-old woman with an aggressive squamous cell carcinoma had beta-HPV-19 integrated into the tumor's DNA and producing viral proteins: the virus was directly involved in sustaining the cancer.
  • The key was a rare genetic abnormality (ZAP-70 protein) that prevented her T lymphocytes from controlling the virus. Having beta-HPV on the skin does NOT cause cancer on its own.
  • A hematopoietic stem cell transplant restored her immunity: the HPV-related conditions — including the carcinoma — disappeared, with no recurrence after more than three years.
  • Ultraviolet radiation remains the primary factor in cutaneous squamous cell carcinoma; carcinogenesis combines genetics, immunity, viruses, and sun exposure, especially in immunocompromised individuals.
  • Final message: there is no reason to fear the normal HPV that lives on our skin, nor to look for it in the general population. What matters is knowing your skin, protecting yourself from the sun, and consulting a doctor about any new lesion that does not heal, grows, bleeds, or changes in appearance.
